The responsibility of placing our puppies in the most loving and nurturing
homes is very serious to us. We realize this questionnaire is intense
to some, even excessive, but we have found the more effort being made
to ensure our puppies are living with a family that is committed to a
long healthy future with their pet, the better opportunity we all have
for success.

Why did you decide on a Labrador retriever for your new pet, and have
you familiarized yourself with the characteristics of the Lab?

Are you certain you are able to afford not only the purchase price of
the puppy/dog, but the ongoing care and maintenance of him/her for the
next 10 - 15 years?

We will fly puppies at your expense, including crate, dishes, airfare
and travel expenses to and from the airport. These fees will need to be
paid prior to flying the puppy out.
If for some reason you are no longer able to keep your Hayshaker puppy/dog
are you willing to return it to us?
Are you willing to provide us with feedback at regular intervals as far
as the puppy's social/physical development and how the puppy is adjusting
to his/her new home? We would like to receive pictures and hear from you!
